Right after you are accepted into a tournament please complete this form

TAP FUNDING GUIDELINES

NEW TAP FUNDING APPLICATION FORM

Travelling hockey teams applying for the Lotteries Yukon Travel Assistance Program (TAP) must send applications to Hockey Yukon, as sport governing body for submission.

*TAP Payments have increased to $400/player this season** FOLLOW THE STEPS BELOW WHEN APPLYING

1. Make a list of the players and coaching staff travelling to a tournament

2. Fill out the form and send the form and list to executivedirector@hockeyyukon.ca

(Player and team staff list can be in the body of the email, they actually prefer that- ensure you place (F) beside female players and staff)

3. Wait for confirmation from Hockey Yukon

4. Submit follow up info as outlined in the Guidelines below.

5. The forms are signed at the bottom by Hockey Yukon President, not you or WMHA

6. The contact info at the top of form is YOUR CONTACT INFO and YOUR POSITION (Team Manager).

Email forms and roster to

executivedirector@hockeyyukon.ca at least 2 weeks prior to travel. Please ensure you get confirmation of receipt and submission from Hockey Yukon.

Submit follow up info as outlined in the Guidelines
- Requirements post event must be received within thirty (30) calendar days to our office by email, fax or in person:
  - Proof of participation for all eligible participants such as the score card/game sheet, results list, tournament officials’ list or adjudication report;
  - High-resolution digital photo(s) of all participants at the event. Email to: lotteriesyukon@yukon.ca; and
  - Proof of Lotteries Yukon recognition such as; thanking Lotteries Yukon on recipient’s social media and/or placing the Lotteries Yukon logo on organization’s website.
